Checking your data in the Clinical Audit Platform
A guide to accessing data in the Clinical Audit Platform for participants in the National Diabetes Foot Care Audit.
Step 1: log into the clinical audit platform
Step 2: go to reporting
Select reporting.
Step 3: select outcome
Select the Outcome report.
Step 4: select the dates for the report
Enter the first assessment date range that you want to view (the default is from the start of the audit to today) and click submit.
Step 5: view all episodes recorded with the time range
This report shows all the episode records for NDFA services for the organisation.
The best way to identify all of yuor organisation's outstanding records would be to click "Download CSV" above the report.
When opened as an Excel sheet, you can filter on the columns "Outcome 12 weeks" and "Outcome 24 weeks" to find the ones that say "Due".
Step 6: add/search for patient records
To go back to add or search patient record, click on "NDFA home", then the "Add/Search for Patient Record". This will allow you to look for the NHS number of the person with missing data and update the record.
Lost To Follow Up
The Lost To Follow Up flag is a new function that has been introduced to help mark patient records which will have no further data recorded against it relating to ulcer healing.
The Lost To Follow Up flag questions apply only to ulcer healing. Other outcomes (such as major or minor amputation) will be determined separately.
The term "Active Ulcer" applies to any wound, including those following surgical treatment of the ulcer if they are not yet healed - and even if it is a wound higher up in the leg, for example, following amputation or after surgical revascularisation.
The activity or inactivity of any associated Charcot disease is not relevant to the assessment.
The Lost To Follow Up flag can be unticked at any time.
