DAPB3085: All Age Continuing Care Data Set (previously NHS Continuing Healthcare (NHS CHC) Patient-Level Data Set)

An information standard which sets out the rules for an end to end, patient level data set that captures data relating to patient details, NHS continuing healthcare eligibility, activity, reviews and care packages.

About this information standard

NHS Continuing Healthcare (NHS CHC) means a package of ongoing care that is arranged and funded solely by the NHS where the individual has been assessed and found to have a ‘primary health need’ as set out in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing in England.

This standard, previously known as the NHS Continuing Healthcare (NHS CHC) Patient-Level Data Set, sets out the rules for an end to end, patient level data set that captures data relating to patient details, NHS CHC eligibility, activity, reviews and care packages.

The existing NHS CHC quarterly collection of aggregate data (DCB2117) will continue to be submitted by providers as normal until the quality of data submitted in DCB3085 reaches an adequate level to allow the quarterly collection to be retired.

This information standard is published under section 250 of the Health and Social Care Act 2012. An Information Standards Notice (see below) provides an overview of scope and implementation timescales and the other listed documents provide further detail for those who have to implement the information standard.

Conformance dates for this information standard are described in the requirements specification and summarised below. 

SERVICE REQUEST RECEIVED TYPE values have been updated to better reflect typical operational practice. This is based on feedback that FNC eligibility is not always indicated on receipt of a CHC checklist, nor is a positive/negative CHC outcome. This is reflected in the specification by removing references to FNC eligibility from the SERVICE REQUEST RECEIVED TYPE item, and replacing code 03 with an "undefined" option.

Further minor corrections have been made to remove references to "CHC" that should have been either "AACC" or not present at all. See the Change Summary tab in the Technical Output Specification for details.


From 1 April 2025 all responsible commissioners of NHS AACC services must

  • be able to collect the information locally to produce the monthly ACC data set
  • ensure their IT systems are able to capture the information for use to produce the monthly AACC data set

From 1 January 2025 AACC system suppliers should update their systems to meet the requirements.

27 April 2021: At the time of publication of version 1.0 of DCB3085 (7 May 2020) the implementation and conformance dates of this standard were noted by the DCB as being subject to change depending on the need for continuing frontline investment in COVID-19 activity. The implementation and conformance dates have been extended by 12 months and the implementation completion date is now 1 April 2022 with full conformance expected by 31 May 2022, in line with the submission of the April 2022 reporting activity. The Requirements Specification and Implementation Guidance have been updated to reflect this.
