DAPB4022: Personalised Care and Support Plan
An Information Standard to establish consistency in information capture regarding personalised care and support planning.
About this information standard
This is an information standard commissioned by NHS England and NHS Improvement from the Professional Record Standards Body (PRSB) with the intent to establish consistency in information capture regarding personalised care and support planning.
The Personalised Care and Support Plan information standard acts as a resource to support the implementation of the targets set out in the Universal Personalised Care strategy document published in 2019.
This first version is to mandate use of the PRSB Personalised Care and Support Plan standard (version 2.0) in appropriate care settings. This will prepare care providers to be able to share relevant information which will be facilitated through a future version of this standard
This information standard is published under section 250 of the Health and Social Care Act 2012. An Information Standards Notice (see below) provides an overview of scope and implementation timescales, and the requirements specification provides further detail for those who have to implement the information standard.

Updates
26 January 2023: The planned dates for work with early adopters have been corrected in the Implementation guide and Requirements Specification to reflect the extended conformance date.
24 November 2022: The full conformance date has been extended from 30 June 2023 to 31 January 2024. The delay is to cater for anticipated winter pressures, create provider implementation materials and resource support for provider sites. Relevant documents have been uplifted to v1.1 to reflect this change.
10 January 2022: Implementation Guidance updated to change "Contingency/Safety Plans" to "Contingency Plans"
