Summary
The learning disabilities health check scheme is designed to encourage practices to identify all patients aged 14 and over with learning disabilities, to maintain a learning disabilities 'health check' register and offer them an annual health check, which will include producing a health action plan.
The learning disabilities health check scheme is one of a number of GP enhanced services. Enhanced services are voluntary reward programmes that cover primary medical services; one of their main aims is to reduce the burden on secondary care services. Data Quality Note
All incorrect codes that were added to the list of SNOMED codes (LD_COD cluster) used to identify people with a learning disability have been rectified in the data for March 2024.
NHS England have been informed of some data quality issues in some local areas associated with the addition of new codes to the learning disability register in December 2023. The codes have been reviewed and confirmed as correct but incorrect coding at local level may have inflated the number of people on the QOF Learning Disability Register in some areas.
One GP Practice has been excluded from the published data for March 2024 due to data quality concerns
