We are aware that for some local areas there may be differences in the number of learning disability health checks recorded in local data and in this publication. Learning disability health checks that are completed at a PCN hub and recorded as happening outside of the GP Practice are not counted in this publication but may be counted in local data. We are looking into a solution but do not have any further information at this time.

Summary
The learning disabilities health check scheme is designed to encourage practices to identify all patients aged 14 and over with learning disabilities, to maintain a learning disabilities 'health check' register and offer them an annual health check, which will include producing a health action plan.
The learning disabilities health check scheme is one of a number of GP enhanced services. Enhanced services are voluntary reward programmes that cover primary medical services; one of their main aims is to reduce the burden on secondary care services. Data for other enhanced services are published annually, the latest release of these data is available under related links below.

267,666 learning disability health checks were completed between April 2024 and March 2025.
This is 79.9% of those on the learning disability register aged 14 and over.
260,722 learning disability health action plans were completed between April 2024 and March 2025.
6,179 GP practices (97.9% of all those active) participated and submitted learning disabilities health check data.
