Summary

The National Diabetes Audit (NDA) continues to provide a comprehensive view of Diabetes Care in England and Wales and measures the effectiveness of diabetes healthcare against NICE Clinical Guidelines and NICE Quality Standards. In July 2008 NICE published guidance for the continuous subcutaneous insulin infusion for the treatment of diabetes mellitus. For the first time in 2013-2014 and 2014-2015 the National Diabetes Audit collected information on the number of people on insulin pump therapy in England.

This collection was a feasibility study to see whether information could be collected on the number and characteristics of people with diabetes using an insulin pump, the reason for going on an insulin pump and the outcomes achieved since starting insulin pump treatment. This National report presents the findings from the feasibility study and details recommendations for the future of the Insulin Pump Audit and also for Specialist Services that participate in the National Diabetes Audit.

The main report contains information on the national key findings and recommendations for England. Supplementary data is also contained in the excel spreadsheets. There are 2 excel spreadsheets; one spreadsheet contains the tables and charts in the report, the other is an interactive excel spreadsheet. Users can select a hospital of choice and the information for the chosen hospital is then displayed in tables and charts.

UPDATE JULY 2016 - It has been discovered that data from some hospitals was not included in the original publication of this report. This has been corrected and the publication has been updated to include this data. The key findings and recommendations from the report remain the same. We apologise for any inconvenience caused.

Highlights

· It was feasible to collect data on insulin pump use as part of the NDA, however further investigation and communication is needed with insulin pump submitters to understand how the process can be improved for future years.

· Either people with Type 1 diabetes are being incorrectly classified as Type 2 or a substantial number of people with Type 2 diabetes are being treated with insulin pumps outside the NICE guidance.

· The reason for starting pump treatment was not completed in a large number of records. Similarly there were frequent gaps in the records of whether treatment goals had been attained. Among those with complete records a slightly higher proportion had started CSII due to problematic hypoglycaemia than to improve glucose control.

· The composite measure of attaining all three NICE treatment targets (HbA1c <58mmol/mol, BP <140/80, total cholesterol <5mmol/l) is more likely to be achieved by people with Type 1 diabetes on insulin pumps than those not on a pump.

· Overall the data shows that pumps may be helping people with diabetes to control their long term blood sugar levels.

 

 

Recommendations

· The National Insulin Pump Audit should be collected again for 2015-2016. HSCIC should work with insulin pump services to help improve the submission process

· HSCIC should review the data items collected and to look at collecting information about individual treatment targets

· Services submitting data to the pump audit should ensure that they are also participating in the full NDA
