Care Identity Service forms

The Care Identity Service forms are used to record information offline, when Care Identity Management is unavailable or not used by an organisation. This information may then be used to enter information into Care Identity Management later.

If you use these forms you will need to keep them for at least 2 years in a secure location, as per NHS England guidance.

The forms are:

• create new user
• request creation of new user
• modify user
• position assignment modification
• cancel smartcard
• RA08 - request to add or remove RA parent link
