Care Identity Service forms
These forms are a post go-live contingency process for Registration Authority (RA) staff in the event of Care Identity Management not being available.
The Care Identity Service forms are used to record information offline, when Care Identity Management is unavailable or not used by an organisation. This information may then be used to enter information into Care Identity Management later.
If you use these forms you will need to keep them for at least 2 years in a secure location, as per NHS England guidance.
