Do not complete the access request form for the Personal Demographic Service FHIR API as you will describe your purpose and use case as part of digital onboarding.

Overview

To request access to PDS data using access methods other than PDS FHIR API:

Follow the steps below. 

Access request form

To access PDS you will need to first to provide a legal basis for viewing the data and explain how you intend to use and access the data by using our online form.  

PDS access use case help

If you are want to submit a new use case then you will need to create a logon if you do not already have one. You should then complete the PDS access new request form.

There is help text in the form. 

Complete the form and submit for review.

If you want to change an existing use case then use the Change to existing Personal Demographics Service (PDS) access form.

If you already have a Service now user account you can use the Personal Demographic Service link.
