Shared Care Record programme

This programme is now closed and has been replaced by the Connecting Care Records programme.

A Shared Care Record was a source of an individual’s past records, and care plans (current and future), connected across multiple health and care organisations accessible in one place. It is based on the person receiving care, not the organisation providing the care. Only authorised individuals involved in that person’s care can access this information.
