Summary Care Records (SCR) are an electronic record of important patient information, created from GP medical records. Use this form to express your interest to view SCR.

Historically, many organisations used Summary Care Record application (SCRa) to access SCR, however this has now been retired. The National Care Records Service (NCRS) is the stand alone successor to SCRa.
