The urgent care self-service tool is new.

We want to make it better.

If you have used the tool when you visited A&E, help us improve it by filling in a quick survey. It will only take a couple of minutes to complete.

Everything you say will remain confidential

If you do provide your contact details (optional) at the end of the survey to allow us to follow up on your feedback as part of our research, your details will be used for research purposes only and you won't be individually identifiable in any data analysis.

We are responsible for your data 

Our privacy statement explains how we keep your data safe. The data from the survey will be shared with NHS Digital staff only. 

Taking part is voluntary 

You can withdraw from the research at any time. You do not need to explain why. Just close the survey in your internet browser.
