Assuring Transformation v4 File upload instructions

1. Status submissions

Submitters of data to ATv4 will be able to bulk-upload patient status submissions in the same way that was permitted under version 3 of the specification: users can either use a blank template (Patient_Status-Submission_Template.csv) into which data can be entered manually or via an automated process prior to upload, or a version 4 "current patient extract" (the “Current Patients Extract AT v.4”, available on the AT Extract Reports page) can re-used as a file upload template (removing and replacing the data as required).

File uploads, whether template based or via a previously downloaded current patient extract must conform to ATv4 file upload requirements. These will be met if a submitter uses the provided file upload template available on the AT data submission guidance web page or re-uses a version 4 current patient extract and (in either case) that the upload file is named as follows: Must be prefixed with “Patient_” (the word “Patient” or “patient”, followed by an underscore) and can optionally be suffixed with any other standard alphanumeric characters, and must end with “.csv”: e.g.: “Patient_Upload_01042024.csv” or simply “Patient_.csv” 

Earlier versions of the file upload template, previously downloaded patient extracts containing only version 3 specification data, an extract containing both v3 and v4 data, or an incorrectly named upload file will not be in the correct format and submitters are unable to use these files to upload new or amended patient status submission data.

Where a valid current patient extract is reused as an upload file, then the fields named patient ID, episode ID and Episode of Care ID must remain in the upload file, but no values entered into these fields will be uploaded.


When adding or editing data to either a template or a downloaded current patient extract, the file must be saved in comma separated value (.csv) format before uploading. Where a submitter is preparing an upload in Microsoft Excel on a Windows PC, the correct .csv file format when saving the document is available under the "Documents" list from the File > Save As menu, as "CSV (Comma delimited) (*.csv)"


2. Restrictive intervention data

Submitters may also bulk upload restrictive intervention data using the template available on the AT data submission guidance web page. Note that versions of this template prior to v4 can no longer be used to upload restrictive intervention data. Restrictive Intervention file uploads are subject to the same file format requirements as a patient status submission upload, but it is not possible to use an extract as the basis for a restrictive intervention upload.

Only an upload file created from the Restrictive Intervention template (Patient_RI_Template.csv), correctly saved as a .csv file and named as follows can be uploaded: Must be prefixed with “Patient_” (the word “Patient” or “patient”, followed by an underscore) and can optionally be suffixed with any other standard alphanumeric characters, and must end with “.csv”: e.g.: “Patient_Upload_01042024.csv” or simply “Patient_.csv”
