The purpose of the Implementation Planning Template is to enable users to plan the relevant activity for implementing the NHS Continuing Healthcare Patient Level Data Set. 

The resulting plan should give a high level picture of how your organisation intends to tackle this implementation to meet the anticipated timescales.

The system conformance checklist is a tool to help service providers assess how well their local IT systems 'map' to the NHS CHC PLDS v1.0 Technical Output Specification.

The NHS CHC PLDS v1.0 Data Model shows each of the individual tables and data items that will make up a data submission. 

 

NHS CHC PLDS User Guidance

The NHS CHC PLDS user guidance document outlines how the national data set should be used and interpreted by users, system suppliers and other stakeholders. This document supports and should be read in conjunction with the Technical Output Specification and Data Model.
