Current chapter – Assurance checklist


These questions have been developed to assist organisations in assessing their current level of data quality assurance and, where applicable, identify gaps in existing assurance processes to focus on. Each question should be scored 1-5 based on the number of ‘Y’ answers against the sub-criteria questions. Any question with an overall score of three or below would suggest an area needing to be addressed using the framework.


1.Oversight

 Do you have executive management sponsorship for data quality and DQ metrics that are presented to and discussed by the board?

To answer this question consider the following sub-criteria questions

Does oversight of data quality exist within the organisation? Yes or No 

Is there someone at a senior level with overall responsibility for data quality and is this reflected in the governance structure of the organisation? Yes or No 

Are DQ data/metrics made available to senior or executive management? Yes or No 

Are DQ inputs required to provide oversight available and subject to sign off? Yes or No 

Are DQ outputs produced and distributed and used within the governance structure as expected? Yes or No 


2. Process

Does oversight of data quality exist within the organisation?

To answer this question consider the following sub-criteria questions

Do explicit data quality processes exist to monitor and manage data quality across the organisation? Yes or No 

Are data quality requirements reflected in clinical and administrative processes? Yes or No 

Are inputs required to support DQ processes available and subject to sign off? Yes or No 

Are outputs from DQ processes produced and used to provide assurance? Yes or No 

Are roles critical to developing and maintaining DQ within clinical and administrative processes in place? Yes or No 


3. People

Do you have dedicated data quality roles and/or defined data quality assurance responsibilities for all staff within your organisation?

To answer this question consider the following sub-criteria questions

Has responsibility for managing and monitoring data quality within the organisation been allocated to specific individuals or a team? Yes or No 

Is this reflected in the roles and responsibilities of those directly involved in managing and monitoring data quality? Yes or No 

Are objectives set and measured through the organisation’s PDR process for those involved in managing and monitoring data quality? Yes or No 

Are generic data quality responsibilities reflected in the job descriptions of all staff directly involved in the capture and upkeep of clinical and administrative data? Yes or No 

Are generic objectives set and measured through the organisation’s PDR process for all staff directly involved in the capture and upkeep of clinical and administrative data? Yes or No 


4. Systems

Do you have clinical information systems configured in line with all relevant Information Standards to assure the quality of data at source?

To answer this question consider the following sub-criteria questions

Are systems configured to validate data at the point of entry?  Yes or No 

Are secondary DQ controls in place to identify/report on DQ issues after the event? Yes or No 

Does the organisation have influence over the application of data quality controls within clinical/administrative systems? Yes or No 

Are inputs required to configure and manage data quality within clinical and administrative systems available and signed off? Yes or No 

Are there system outputs produced that can be used to provide quality assurance? Yes or No 


5. Measures

Do you actively use external data quality metrics such as NHS England's DQMI to assess and improve data quality locally?

To answer this question consider the following sub-criteria questions

Have all externally available data quality metrics and reports been identified? Yes or No 

Are external DQ metrics and supporting data reports downloaded and made available to those responsible for data quality assurance activities? Yes or No 

Are external DQ metrics used to identify data quality issues and support tracking of improvement activities? Yes or No 

Are external DQ metrics used to support staff awareness and inform training? Yes or No 

Are external DQ metrics used to support executive oversight activities?  Yes or No
