How we use the information (processing activities)

The All Age Continuing Care (AACC) data set is a patient level, output based, secondary uses data set which aims to deliver robust, comprehensive, nationally consistent and comparable person-based information for children, young people and adults who are in contact with continuing care services, as defined in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care, and the National Framework for Children and Young People’s Continuing Care. As a secondary uses data set it intends to re-use operational and financial data for purposes other than direct patient care.

The AACC data set is used to inform service improvements and monitor service performance, patient experience, and outcomes.

This is an end-to-end data set (from referral to end of service provision) covering initial checklist all the way through the assessment process to the commissioning and monitoring of individual care packages.

Processing of information is key in turning raw data into understandable intelligence on all aspects of performance, patient care and efficiency relating to NHS AACC

How long the data is kept A maximum of 20 years, or dependent also on continuing validity of Directions or as per the current NHS Digital Records Management Policy, should the retention period change. The data will be appraised before it reaches its maximum retention period in accordance with the NHS Digital Policy and either recommended for secure destruction or an extension of the retention period if there is a justifiable reason.

Is the data subject to decisions made solely by computers? (automated decision making) The processing is not subject to any automated decision making.
Where does this data come from? NHS England collects the NHS All Age Continuing Care data from all Integrated Care Boards (ICBs) that commission both NHS funded and jointly funded AACC services in England (and any Commissioning Support Units (CSUs) providing this service on behalf of ICBs).
The legal basis for collecting this data

Direction from NHS England under s.254 of the Health and Social Care Act 2012 to establish and operate a system for the collection and analysis of Continuing Healthcare Patient Level Data. The NHS All Age Continuing Care Directions 2024 (which replaced The Continuing Healthcare Directions 2022 which were previously relied upon) are published on the NHS England website.
