Quality and Outcomes Framework, 2024-25


Summary

The objective of the Quality and Outcomes Framework (QOF) is to improve the care patients are given by rewarding practices for the quality of care they provide to their patients, based on several indicators across a range of key areas of clinical care and public health. This publication provides data for the reporting year 1 April 2024 to 31 March 2025 and covers all General Practices in England that participated in QOF in 2024-25.

Payment protection has been applied to some indicators in 2024-25. More information on payment protection can be found in the ‘Technical Annex’ which forms part of this publication and QOF guidance found under 'Related Links'.

GP practices have been mapped to their respective PCNs, Sub ICB Locations, ICBs and Regions using reference data current on 1 April 2025. This mapping has been applied to data for both the current and previous reporting year.

Please note, the ‘Indicator definitions.xlsx’ file has been discontinued. This information (indicator IDs, descriptions and point values) is still available in the ‘MAPPING_INDICATORS.csv’ file, which can be accessed via the ‘Raw data.csv’ files download from the Resources section below.

6,188 practices included in the 2024-25 publication

Participation in QOF is voluntary, though participation rates are very high, with coverage of 97.6% this year.

QOF recorded prevalence rates

Hypertension (15.2%), depression (14.3%) and obesity (13.9%) had the highest QOF recorded prevalence rates.

Changes in QOF recorded prevalence

Obesity had the greatest change in QOF recorded prevalence, which was 1.1 percentage points higher than in 2023-24.

Note – 2023-24 prevalence rates have been recalculated using data for practices included in QOF 2024-25 only.

Practice achievement

5,149 (83.2%) of practices achieved over 90% of the available QOF points.

Value of a QOF point

A QOF point had a value of £220.62, an increase of 3.4% compared to 2023-24.

Personalised care adjustment rates

The chronic obstructive pulmonary disease (COPD) indicator group had the highest personalised care adjustment (PCA) rate at 29.9%. Smoking had the lowest PCA rate at 1.8%.

Patients newly diagnosed* with depression

The number of patients aged 18 and over, newly diagnosed with depression was 714,592, a decrease of 4.2% compared to 2023-24.

*Patients are classed as newly diagnosed if their latest first or new episode of depression is within the period 1 April – 31 March of the relevant QOF year, or if their latest first or new episode of depression was in the 1 January – 31 March period of the previous year, with no depression review by 31 March of that year.
