COSD change request form

This form has been created to support front line staff and suppliers, make suggestions for change during the life-cycle of a version. Please note that once a data set has been approved for change by the Data Alliance Partnership Board (on behalf of the Secretary of State for Health and Social Care), it cannot be changed until the next formal review and version change.

All suggestions through this process will be collected and submitted for discussion through the COSD Advisory and Governance board and if approved, will form part of the next change process.
