GP Connect: Send Document
GP Connect: Send Document provides the capability for a patient’s registered GP practice to receive a document capturing the details of a care encounter following a patient being seen in another care setting.
Sending information via GP Connect: Send Document
GP Connect: Send Document provides the capability for a patient’s registered GP practice to receive a document, such as a consultation summary from another care setting, which captures the details of a care encounter. The product uses the MESH file transfer service. The PDF is created within the sending organisation's system, transferred across secure infrastructure, and delivered directly into the receiver system where it can be processed and filed. This can replace similar information being sent via other methods such as printed letters or NHS Mail and reduces administrative time and effort to send the consultation.
The current version of GP Connect: Send Document allows GP practices to receive a consultation summary as a PDF, which can then be saved to the patient record by the practice.
The next release of GP Connect: Send Document provides the ability for care services to send other documents and file types to practices, which will be described to inform the practice what type of document has been sent.
Examples of where this product is used
GP Connect: Send Document is used within Primary Care Networks (PCNs) to send consultation summaries back to the registered GP.

GP Connect: Send Document may also be used by some secondary care providers to send back a summary letter or medical communication which provides an update to the registered GP.
Working with GP Connect: Send Document
Where the updated version of GP Connect: Send Document is deployed the GP practice (receiver) will be able to:
- identify the type of document being sent
- identify the care setting in which the activity took place (if known)
- obtain an optional SNOMED code to provide further context about the event, which may help the practice code the event in the record
Enabling GP Connect: Send Document
Practices receive the document without the need for additional software. Senders can develop the product within their software to automatically send an update following an episode of care.
If your care setting wants to use GP Connect: Send Document, you should first check that your supplier has developed the product - see the list of suppliers. Your supplier should support end-user organisations with configuration and training. Documents that are sent via GP Connect: Send Document use a SNOMED code from an approved list of codes.
