Below shows the questions asked in the expression of interest form. The NHS App team use this form to gain a better understanding of your product.

If you want to integrate with the NHS App and you meet our eligibility criteria, submit an NHS App expression of interest form.

Questions with an asterisk (*) require an answer.


Section 1: Your contact information

1. What is your company name? *

2. What is the name of your product(s) or service(s) that you would like to integrate with the NHS App? *

3. Please give an overview of your product. You can give a website link here is that supports the overview.*

4. Please provide a named contact for your company or department. *

5. Please provide their email address. *


Section 2: Product functionality

6. Do you provide a product or service for patients registered at a GP Surgery in England? (Yes/No) *

7. Do you provide a product or service to patients who are receiving secondary care services in England? (Yes/No) *

8. Who are the users of your product or service? (for example general population, people with diabetes, post-operative patients) *

9. Currently how many patients are registered to use your product or service? *

10. How many of your registered patients are active users of your product or service? *

11. Is your product or service free to the patient at the point of delivery? This excludes any NHS-levied charges, such as prescription charges. (Yes/No) *

12. Do you have a public-facing Service Desk? (Yes/No) *

Section 4: Design and delivery

Section 6: Read and commit

35. By ticking this box you confirm that you have checked your product is eligible for integration with the NHS App. You will need to meet the following standards and requirements on the NHS App website. *

  • standards for NHS App integration: WCAG 2.1, NHS service standard, clinical safety standards and data privacy standards
  • having dedicated design resources available during the integration
  • having a dedicated development team available during the integration process

We confirm that our product is eligible and we will be able to meet the standards and requirements specified above.
