If you need additional information from a patient
The questions we ask in the online registration form are all you need to progress a patient registration.
Patients cannot leave fields unanswered as the service uses data validation. You should have all the information you need to progress their registration, even if there are unanswered questions.
Follow your surgery's standard process for asking for additional information if you need this from a patient.
If a patient cannot provide a permanent address
An individual does not need personal identification (ID), an NHS number or proof of address to register with a GP surgery. If they do not have a permanent address you can still register them using a temporary address or the address of your surgery.
