Adding extra questions
Add extra questions to the online form for any additional services your practice might offer.
Add extra questions to the form when you register through Profile Manager. These questions relate to additional services some practices offer. Patients will be asked them in addition to the standard questions asked in the service.
These questions ask patients if they:
- want to get their prescription items directly from your practice, if you are a dispensing surgery
- need an interpreter at their appointments
- want to nominate a pharmacy to collect their prescriptions from
- are university students
- are eligible for tuberculosis screening
You can manage what questions you ask patients in NHS Profile Manager.
Direct dispensing
You can ask this question if you are a dispensing practice. The service will check if the patient is eligible for direct dispensing services.
They must tell us that they:
- want to get their medicines or medical aids direct from your surgery
- live more than 1 mile from their nearest pharmacy, or
- would have serious difficulty getting to their nearest pharmacy
Depending on how the patient answers we will tell them that they:
- are not eligible for the service
- may be eligible for the service and that you might discuss it with them
Interpreting services
Turn this question on to ask your patient if they need an interpreter at their appointments.
The service will ask the patient:
- if they need a language interpreter
- which language they need an interpreter for
Nominating a pharmacy
Turn this question on to let your patients nominate a pharmacy.
The service will ask the patient if they would like to nominate a pharmacy for their prescriptions.
If the patient answers yes, they will be asked:
- if they prefer an online or high street pharmacy
- to search for and select a high street pharmacy using a postcode look up, or
- to register directly with an online pharmacy
Identifying university students
Turn this question on if you are a university-affiliated practice or you register mostly university students at your surgery.
The patient will be asked:
- if they are a student at a UK university
- for their student identification number
- the year their course finishes
- where they are living while at university
Screening for tuberculosis
Turn this question on if you want to offer tuberculosis (TB) screening to patients.
The service will identify patients that:
- are aged between 16 and 35
- have recently arrived in the UK from a country outside the EU
- were born in a high risk country or may have spent more than 6 months in a country where there was an increased risk of catching TB
For these patients a flag will show on their registration details indicating they may be eligible for screening.
