Complete this form to request an NHS England Terminology Server system-to-system account.

This form will ask you to select your purpose for requesting a Terminology Server system-to-system account. Ensure you have read the definitions on the previous page.

We will also ask you to confirm you have read the system-to-system account agreement.

Purpose for accessing Terminology Server content

Be aware that Direct Care access requests may take longer for us to process, due to additional checks and permissions that are required. Please provide as much information as you can when prompted to avoid any delay.

Direct Care (Production 2)

It is important to note that Direct Care access is not granted until testing has been completed and the system is ready for live. The Direct Care (Production 2) environment is preserved solely for real-time direct care scenarios. Due to this we need to ensure it is not used until full testing has been completed. This will also need to be confirmed.

Production environment 1 is reserved for testing, consumer and syndication use. Production environment 2 is preserved for real-time direct care clinical use.

Please confirm if testing has been completed in Production 1.
We need to know this prior to setting you up on the clinical environment.

For suppliers who have completed testing and are applying for Direct Care (Production 2) access we also need the following information.
