Safeguarding Case Review Tracker

Data Provision Notice requiring submission of data from integrated care boards which will enable the case tracking of Serious Incidents that have resulted from safeguarding failures.

Overview

The purpose of this data collection is to provide a facility for recording Safeguarding Case Reviews in health care which have occurred as a result of a safeguarding failure. When safeguarding cases occur, they must be reported so they can be identified correctly, investigated thoroughly and trigger actions that will prevent them from happening again. 

In broad terms, Serious Incidents are events in health care where the potential for learning is so great, or the consequences to patients, families and carers, staff or organisations are so significant, that they warrant using additional resources to mount a comprehensive response. Serious Incidents can extend beyond situations which affect patients directly and include incidents which may indirectly impact patient safety or an organisation’s ability to deliver ongoing healthcare.

NHS England provides a Safeguarding Case Review Tracker that enables the collection and analysis of information about Serious Incidents that have resulted from safeguarding failures. The data will be submitted into the Safeguarding Case Review Tracker by integrated care boards (ICBs) in England.


Scope of the collection

ICBs in England. It is anticipated that the designated safeguarding nurse/lead in the ICB will input data into the safeguarding tracker system.

Period of the collection

This will be an ongoing collection and therefore the frequency of submission will be as needed, with records  being updated in real time with new information.

Each submitting ICB will access NHS England’s Forms Platform to record and submit data. ICBs will then be given access to the Safeguarding Case Review Tracker in order to edit and review their submitted data.

Updates

19 March 2025: Data will be made available to ICBs, regional teams and providers via the NHS Safeguarding Integrated Data Dashboard (NHS SIDD).
