Impact of Cyber Incident on MHSDS data submissions

Unfortunately, a number of providers of MHSDS data have been affected by a recent cyber incident.  NHS Digital has actively worked with these providers to understand how best to resolve any data submission issues that may have occurred as a result.  We also continue to encourage affected providers to reach out to [email protected] as well as their regional Emergency Preparedness, Resilience and Response (EPRR) teams.

As of Performance June 2022, the impact on monthly MHSDS data appears to be quite limited.  There may be a greater impact on subsequent months’ publications so we will monitor this closely and introduce contingency measures if required.  No changes to submission processes will be introduced while service providers are actively dealing with this issue.

Providers will also have the opportunity to resubmit MHSDS data for any month between April 2022 and February 2023, up until the final 2022/23 multiple submission window model (MSWM) deadline in May 2023, and this should serve to further mitigate the issue.  This will then feed into the 'End of Year Final' data made available shortly afterwards.

When interpreting the Performance June 2022 data, it is also important to be aware that there was a double bank holiday for the Platinum Jubilee, and so fewer working days in June 2022 than usual.

Error affecting single breakdown within the Performance June 2022 Restrictive Interventions CSV file

An error has been identified in the data contained within the ‘England; Restrictive Intervention Type; Age Group; Upper Ethnicity’ breakdown of the MHSDS Monthly: Performance June 2022 Restrictive Interventions CSV file.  This resulted in incorrect figures being shown for measures MHS76 and MHS96, and it also caused the omission of figures for MHS77.

The error has now been rectified and corrected figures are available within the MHSDS Monthly: Performance June 2022 Restrictive Interventions V2 CSV file.

NHS Digital apologises for any inconvenience.

Resolution of two historic issues affecting MHSDS Monthly: Time Series Data for Selected MHSDS Measures, April 2016 to Performance June 2022 CSV File

Two historic issues have been identified affecting the MHSDS Monthly: Time Series Data for Selected MHSDS Measures, April 2016 to Performance June 2022 CSV File.

  1. End of Year Final April and May 2022 data for ED87e (Proportion of referrals with eating disorders categorized as routine cases entering treatment within four weeks in RP, aged 0-18) was found to be incorrect.
  2. End of Year Final June 2022 data for Children and Young People with an Eating Disorder metrics (those in the ED85 - ED90d range) contained some categories within the 'Breakdown' column.  Rows which should have been assigned a Breakdown category of 'Provider' or 'STP' were erroneously assigned a category of 'CCG - GP Practice or Residence'.

Both issues have now been resolved and revised data are available within MHSDS Monthly: Time Series Data for Selected MHSDS Measures, April 2016 to Performance June 2022 V2 under 'Resources'. NHS Digital apologises for any inconvenience caused.

Issue affecting MHSDS Monthly: Performance June 2022 Data Quality VODIM and Integrity File

The MHSDS Monthly: Performance June 2022 Data Quality VODIM and Integrity CSV File has been temporarily removed. This is due to an error in the code for the Organisation Identifier (Code of Commissioner) measure, resulting in valid codes being flagged as invalid. The new VODIM extract will be re-issued once the error has been corrected.  NHS Digital apologises for any inconvenience caused.

Notes on Measures

Establishment of Integrated Care Boards (ICBs) / Integrated Care Partnerships (ICPs) and abolition of CCGs and STPs as of July 2022

Integrated Care Systems (ICSs) were formally established across the NHS in England on 1 July 2022, with the introduction of Integrated Care Boards (ICBs) and Integrated Care Partnerships (ICPs), and abolition of CCGs.

Further information around the introduction of ICBs and ICPs is available via the relevant Organisation Data Service (ODS) is available.

Breakdowns relating to commissioning structures as they existed prior to 1 July 2022 (CCG, STP, Region and Commissioning Region) are reflected in the Performance June 2022 data as normal.

The derivations necessary to support reporting under the new commissioning structures are underway but are yet to be completed.  As a result, it has not been possible to include ICB / sub-ICB breakdowns in the Provisional July 2022 data released as part of this publication.

Furthermore, breakdowns relating to the legacy commissioning structures have been omitted from the 'MHSDS Monthly: Provisional July 2022 MHSDS Data' CSV file.  Similarly, tables 1b, 2b, 3b and 5b been redacted from the 'MHSDS Monthly: Provisional May 2022 to July 2022 Access and Waiting Times Reference Tables'.  It was not considered appropriate to keep these in, given that they were no longer current as of 1 July 2022.

NHS Digital will include reporting on the new commissioning structures in all monthly MHSDS publications at the earliest opportunity.  Should delays persist into next month or beyond, we will seek to report on these new commissioning structures retrospectively for any months for Performance data from July 2022 onwards once in a position to do so.

Changing existing measures

Methodological improvement for MHS29d and MHS30f

  • MHS29d - Contacts in the RP with community mental health services for adult and older adults with severe mental illness
  • MHS30f - Attended contacts in the RP with community mental health services for adult and older adults with severe mental illness

has been improved to align with other care contact measures for consistency.

At national level, the change in methodology results in a 3.1% decrease for MHS29d and a 3.2% decrease for MHS30f.

Announcement of methodological changes to the Restrictive Intervention Statistics

From Performance April 2022 onwards, the methodology underpinning construction of the restrictive interventions measures has changed.  This was necessary due to the changes within the restrictive interventions dataset tables introduced in MHSDS v5.  The methodological change review involving NHS England, CQC and NHS Digital resulted in an agreed new methodology to calculate the existing set of measures.

For further details, please refer to the Announcement of methodological changes to the Restrictive Intervention Statistics published on 29 June 2022 as well as the the metadata file.

Please also note there is a known DQ issue affecting MHSDS v5 data (October 2021 data onwards) allowing providers to submit restrictive intervention ‘incident’ data to the MHS505 table without specifying a corresponding restrictive intervention ‘type’ in the MHS515 table.

Incident data which does note have the corresponding restrictive intervention ‘type’ data is not counted as part of our current restrictive intervention measures, as these currently only use data from the MHS515 table.

We are working with providers to resolve this issue.

MHSDS version change

The move to MHSDS version 5 from the October 2021 reporting period onwards brings with it changes to the dataset, and the construction of some metrics has changed as a result.  In particular, the way in which contacts are reported has changed to allow a more detailed view of how patients interact with services.  Some of the codes used in version 4.1 are now invalid yet continue to flow, and this has caused issues with metrics which used these fields such as the CYP access measures, as well as measures which show a breakdown by Consultation Mechanism.

Additionally, changes have been made to the way in which the Clinical Response Priority for referrals is reported.  A new code for ‘Very Urgent referrals’ has been added, and this is now used in the methodology of metrics which use this field.

Furthermore, where a code was previously valid in version 4.1 but is no longer valid in version 5, NHS Digital has contacted providers to try and ensure that future submissions of the dataset are rectified to only include codes that are valid. This is particularly in relation to the Consultation Mechanism for care contacts and the service or team type referred to for referrals.

Full details of the changes to the dataset are available in the Technical Output Specification.
