Manual completion of the template
Complete all the fields within the spreadsheet template.
When populating the required data fields. If you do not complete all of the mandatory fields for a patient, then that record will be rejected.
- remove any spaces in the NHS number field
- for the date field, the format is DD/MM/YYYY
- for the offered and attended fields, 1 should be entered for Carried out and 2 should be entered for Not done
- If 1 (Carried out) is entered in the offered and attending fields, the corresponding observation date then becomes a mandatory field
File structure
Files are made up of variable length records, each record ending with a carriage return and line feed (ASCII 13, 10).
The data in each record is split into fields by using a comma separator (“,”). Multiple values for one field need to be separated by a semi-colon in the file (not a comma). The standard is that commas separate the individual fields and semi-colons separate multiple values within a single field (where appropriate).
The last line entry should not have a comma afterwards. The file name needs to be formatted as follows:
patienteducation_
Other useful information can be added after the underscore if helpful to the user for example date or organisation (as CAP will not read after the underscore) for example patienteducation_February
Data quality
The validation process includes screening of the values submitted to determine if they are within an expected range. If the values fall outside this expected range the warning flag is raised and the relevant error message will be reported to the submitter in the File submission details page.
Creating and saving your csv files
Make sure you have saved the Spreadsheet onto your computer before entering any data. We recommend either the Downloads folder or the My Documents folder, or equivalent. Somewhere that is easily accessible for you.
