Add data to the record tree

Once the patient record has been entered, the Record Tree will be displayed.

For all sections below, clicking Submit will generate a You have successfully added a record message and will add the record to the record tree for that section.

See the end of the NICE Processes of Care Data section for an example. This same functionality applies to all sections. You can edit or delete records from the record tree, and the most recent entry will appear at the bottom of the list.

Click on Add record next to NICE Processes of Care which will open up the options to enter data for the 5 NICE Care Guidelines. 

Click on the Serum Creatinine tab, enter the Level and click Submit. Enter date of observation and click Submit.

The Serum Creatinine Level must be between 20 and 1000.

Click on the Urinary Albumin tab and enter the Level (which will bring up the date field). Select the testing method and stage from the drop down options and click Submit.

Urinary Albumin Level must be in the format: 0000.0.

Click on the Eye Examinations tab and select whether a review was carried out using the drop-down option. Enter the Observation Date if selecting 'Carried out' and click Update.

Click on the Foot Examinations tab and select whether a review was carried out using the drop-down options. Enter the Observation Date if selecting 'Carried out' and click Update.

Click on the Smoking Status tab and choose the most appropriate option from the drop-down list, enter the Observation Date and click Update. 

Once you have submitted any or all of the 5 items, it will display the message below.

The observation dates of the records you created will appear on the Record Tree. You are able to edit or delete this record if necessary.

If you wish to add a new record for all items or just an individual process, the record(s) will be displayed with the most recent update at the bottom of the list:

Click Add record to add HbA1c information.

Add the HbA1c Level and Observation Date and click Submit.

HbA1c Level must be between 3.9 and 195.1.

Click Add Record to add Blood Pressure information.

Add figures for Systolic and Diastolic Blood Pressure and the Observation Date, and click Submit.

Systolic BP must be between 70 and 300, Diastolic BP must be between 20 and 150.

Click Add Record to add BMI information.

Add the BMI value and the Observation Date and click Submit. 

BMI must be between 16 and 70. 

Click Add Record to add Cholesterol information. 

Add the Total Serum Cholesterol Level and the Observation Date and click Submit. 

Cholesterol level must be between 1 and 40. 

Click ‘Add Record’ to add Clarke and Gold information.

Add Clarke Score and Observation Date and click Submit. 

Add Gold Score and Observation Date and click Submit.
