Add data to the record tree glucose technology and education review
Add glucose technologies information
Click Add record to add Glucose Technologies information.
Add whether the patient is on an open loop insulin pump and the Observation Date.
If selecting '01: Yes- NHS Funded' or '03: Yes- Non-NHS funded', enter the Date started using loop pump and click Submit.
If No is selected, enter the Observation Date and click Submit.
Click the rtCGMS tab to add whether the patient is using a rtCGMS and the Observation Date.
If selecting '01: Yes- NHS Funded' or '03: Yes- Non-NHS funded', enter the Date started using an rtCGMS and click Submit.
If No is selected, enter the Observation Date and click Submit.
Click the Closed Loop tab to add whether the patient is on a Closed Loop System and the Observation Date.
If selecting '01: Yes- NHS Funded' or '02: Yes- Non-NHS funded', enter the Date started using Closed Loop System and click Submit.
If No- Offered but declined is selected, enter the Observation Date and click Submit.
A confirmation message will be displayed on screen to confirm you have successfully submitted the record.
Once you have clicked Submit, the Observation Date, with the option to Edit or Delete the record will be displayed on the Record Tree:
If you wish to add a new record, each individual record will be displayed with the most recent entry at the bottom of the list.
Add patient education review information
Click Add record to add patient education information.
Select from the drop down box whether a Patient Education Review has been carried out. If 'Carried out' is selected, enter the Observation Date.
Next, select whether a Diabetes Structured Education Programme has been offered and attended and select ‘Carried out’ or ‘Not done’. If ‘Carried out’ is selected, enter the Observation Date and click Submit.
