CYP105 Onward Referral
Every CYP105 Onward Referral record must have an associated CYP101 referral record included in a submission file. It is expected that this field would be used most frequently by tier 3 weight management services to report the onward referral of a patient to another services, for instance a tier 4 provider or for psychological therapy.
SERVICE REQUEST IDENTIFIER
This is a mandated data item; the record will be rejected if it is not included within this group.
This data item is used for linkage back to the CYP101 Service or Team Referral group.
ONWARD REFERRAL DATE
This is a mandated data item; the record will be rejected if it is not included within this group.
The date the patient was referred from one service to another service, which may be in the same or a different organisation.
ONWARD REFERRAL REASON (COMMUNITY CARE)
The reason why the person was referred from one service to another service, which may be in the same or a different organisation.
It is expected that this field would be used most frequently by tier 3 weight management services to report the onward referral of a patient to another services, for instance a tier 4 provider or for psychological therapy.
Where referral is to:
- a tier 4 provider please use ’01 Transfer of Clinical Responsibility’;
- psychological therapy please use ‘For Opinion Only’
- for another intervention please use ‘Other’
The above onward referrals can also be captured within the CYP202 Care Activity table as a Coded Procedure. Please see section on CYP202 Care Activity for appropriate SNOMED CT codes.
ORGANISATION IDENTIFIER (RECEIVING ORGANISATION)
The ORGANISATION IDENTIFIER of the ORGANISATION that is receiving the PATIENT from another Health Care Provider.
