Every CYP602 Disability Type record must have an associated CYP001 patient record included in a submission file.

LOCAL PATIENT IDENTIFIER (EXTENDED)

This is a mandated data item; the record will be rejected if it is not included within this group.

This data item is used for linkage back to the CYP001 Master Patient Index and Risk Indicators group.

DISABILITY CODE

An indication of whether a PERSON is disabled.

Under the Disability Discrimination Act (DDA), a disabled PERSON  is defined as "someone who has a physical or mental impairment that has a substantial and long-term adverse effect on his or her ability to carry out normal day-to-day activities".

DISABILITY IMPACT PERCEPTION

This data item is not required for the NOA.
