When a patient goes on extended S17 leave (linked to a legal judgment, for example MM), commissioners will need to:

  1. Ensure the current open record on AT is up to date to reflect the position before the patient goes on extended S17 leave.
  2. Close the existing record, with Date of Closure of Record Q49 being the date the patient went on extended S17 leave.
  3. Open a new record with most details the same as the closed record, to cover the period when the patient is on extended S17 leave.
  4. Maintain the record – minimal updating should be required whilst the patient remains on leave.

If a patient on extended S17 leave is recalled to hospital and remains there, the commissioner will need to:

  1. Close the existing record that reflects the period of leave, with Date of Closure of Record Q49 being the date the patient came back into hospital.
  2. Open a new record with most details the same as the previous record for their return to hospital.
