Current chapter – Other KO41b form information


This section includes examples, functionality or common error messages that may be encountered when completing the KO41b return.


Subject Area and Staff Group – two stage data entry process 

In these 2 sections a value does not need to be entered into every box. Only enter data in the required boxes (in this example 2 complaints for the Communications subject field).

First, tick the required box, which drops down the value box:

If you delete the value you will see an error as follows:

To resolve this error, either enter a value in the box, or untick the box.


Common error messages

The 'Total Carried Forward' value must always be a positive value. The return below cannot be advanced because a total of 7 complaints have been entered in Number Upheld (2) + Number Partially Upheld (0) + Number Not Upheld (5), even though there are only 5 'Total New Complaints'. In this example 'Total Resolved' must be 5 or less.

Summary information - common error message

In the summary information, 'Age of Patient' and 'Status of Complainant' sections a value must be entered in each box. If a field is left blank an error will appear when you try to save and you cannot advance until a value is entered. Enter 0 (zero) if you have no other value to enter. On the example below there are 5 'New Complaints' and 5 values have been entered in the form, but a 0 would still need entering in the 'Age Unknown' field.

In the 'Age of Patient' and 'Status of Complainant' sections the component parts must equal the value entered in the 'Total New Complaints' box. The error below has occurred because the 'Age of Patients' equal 6 when they should equal 5, as 5 was the number of 'Total New Complaints' entered in the 'Summary Information' section.

In the 'Subject Area' and 'Staff Group' parts of the return the total components must be equal to or greater than the 'Total New Complaints' value. The error below has occurred because only 'Subject' values equalling 3 have been entered, but there are 5 'New Complaints'. To resolve this at least 2 more values need to be entered.

SDCS submission status

You may notice that after you have completed and submitted your KO41b return that the 'Status' field on the home page of the SDCS collection system remains as 'Closed/Not Submitted'.

This is a known issue and is a result of there being no direct link between this SDCS page and KO41b collection form itself. Therefore, if the KO41b has been completed as described in the 'Completing the KO41b data submission' section of this guidance then the return is complete and submitted to NHS England, and the Status message on the SDCS page can be ignored.
