Part of Primary care written complaints guidance notes KO41b – GP collection
Submitting data
How to access SDCS and the KO41b form
Each GP practice will be invited by email to make a GP primary care complaints return on the NHS England Strategic Data Collection System (SDCS).
If practices have submitted data before for the KO41b, eDec or the new National Workforce Reporting Service (NWRS), the login will use the same credentials.
If you have not used SDCS previously please click Register on the login page and complete the registration form.
Once you have registered you should see the following dashboard.
To access the KO41b form please click Submissions and then New.
Select KO41b GP complaints from the collection dropdown and click the Create button.
Once you have clicked Create you will be taken to the KO41b form. Select the practice you are submitting data for and then click Continue. If you are registered for more than one practice you will have to submit data for each individually. (Most users are likely only to be registered for one practice).
When you have chosen your practice select GP Complaints (KO41B) 2024 -2025 and click Continue.
The following example screenshots show a practice called ‘TEAMS MEDICAL PRACTICE’ with 5 new complaints in 2024 - 2025.
To begin the return select the green Create New Record button, or also select Start under the Action heading.
You will then be able to proceed through each section of the KO41b form. There are 5 separate sections which require data:
- Summary Information
- Age of Patient
- Status of Complainant
- Subject Area
- Staff Group
At the end of each section press Save and continue to proceed to the next section.
