The National Diabetes Foot Audit (NDFA) team with Diabetes UK produce a patient information leaflet. The leaflet has been developed with input from patients. The leaflet should be handed out to all eligible participants preferably at their first appointment. You should discuss the aims of the audit and allow them time to ask questions about the audit.
If the patient expresses an interest for their data to not be included do not enter their information into the Clinical Audit Platform. If after data entry the patient contacts you and asks to be removed please delete their record and notify NHS Digital so that we can remove from any subsequent analysis.
For Welsh healthcare services
Once the patient has had time to review the patient leaflet informed consent should be obtained using the NDFA consent form. Only once consent has been obtained can data be submitted to the clinical audit platform.
