Using the online portal and data collection application

NHS England will facilitate the setting up of new users on the portal. Once you have a login, you can access the portal by going to the NHS England applications webpage: https://apps.model.nhs.uk

Here, click login and sign in with your email address and password. On this homepage, you can navigate to my account via your name in the top right corner, to amend your personal details or password if needed.

Once you’ve logged in to the NHS England Applications portal, navigate to P and select Perinatal Pathology Data Collection. Please note, you will see other applications listed here as some are open to all NHS staff.

Once you’ve selected Perinatal Pathology Data Collection You will be redirected to the data collection area, where you should see Perinatal Pathology with a small plus sign next to it.

Clicking the small plus sign will allow you to navigate to this week’s form. The submission window opens on a Monday for 4 days, closing on Thursday. Where there is a Monday bank holiday, the submission will still close on Thursday. The submission window opens and closes automatically.  The weeks are numbered 1-52, starting in April. Once the submission window opens you can click start to make your submission. It may take a moment to load the next screen after you click start.
