Incident occurs

Provider or lead commissioner to log SI onto STEIS.STEIS then generates a secure email to NHS England and Improvement generic inbox at locality. If not recorded on STEIS the incident should be alerted to the designated lead in the relevant CCG. This will include primary care incidents. 

Confirm level of investigation required which may include a strategy meeting to confirm route of investigation. 

Path 1

Panel meeting agree the requirement for a case review: CSPR, SAR or DHR.

All information will need to be included in the tracker in the relevant fields.

Online tracker to be updated as new information or actions as they occur. 

Tracker information to be discussed at relevant ICS meetings. 

ICS to provide a pre-publication briefing to local communications team and escalation if potential media attention.

Update tracker when case is closed (published or embargoed).

Path 2

Follow the serious incident management framework process. Appendix 3.

NHSEI Internal notifications any serious safeguarding incidents that may potentially involve media interest must be flagged to the NHS England local office team via the safeguarding lead who will report onto

  • director of commissioning operations
  • director of nursing
  • medical director
  • locality director
  • communications team via local teams if media attention

If any of the incidents involve a specialist commissioned patient the communications team will notify the specialist commissioned communications team.

Escalation route to regional/national team through the regional safeguarding lead. 

The regional safeguarding team will support dissemination of information and system wide learning.
