Submitting data

Recording data
  • Include case reviews including the StEIS number and local reference number, if they are available. 
  • When using the open and closed definitions use 'closed' as published or embargoed if the review is completed and signed off. 
  • When recording the date of the incident into the Tracker, enter the date the incident occurred. Once a decision has been made to investigate or review the case, use this as the "date added" to the tracker. 
  • Use the DHR child for a 16/17 year old child. 
  • When using the tracker please use google chrome or firefox to open the browser. 

Recording dates
  • The report signed off date should be the date the chair of the independent review or panel signed off the report. 
  • Its up to each region to decide how many historical "closed" case data they want to enter into the tracker. 
  • Any new cases from 1 April 2021 should be added to the tracker and any historic "open" cases from before then should also be added. 
Using the comments box
  • If the case you are entering does include the relevant "type of review" in the drop down options,  please use the comments box to record this information. 
  • Use the comments box to record any further updates or comments you may want to include including the type of review or further information about the child or parents.
