1. Logging in

You can log into the Safeguarding Case Review Tracker using the following link. We recommend using Google Chrome or Firefox as your browser.

Select the Safeguarding Case Review Tracker option from the drop down list and then select the create iconNote, you should also use this option for viewing or editing an existing record in the Tracker.

Create a new record by selecting the green button create new record.  


2. Recording information

Enter the basic case details of the incident. Include the StEIS number and any local reference number if they are available.  

Use open if the case is yet to published and closed if it has been published embargoed or if the review is completed and signed off.

Date added- this refers to the date the referral was made into the Safeguarding Case Review Board on the referral form, and should be the date the record is added to the tracker.

Chose from the drop down options to complete the designated professional field.

Enter the name of the Safeguarding Partnership Board and the identifier type(s) for example the StEIS number, and/or any local identifier used.

Use the case information section to type in basic case information.

The options for the abuse vulnerability themes 1, 2 or 3 are the same so select from the relevant themes

When you are using the type of review section use converted when a Rapid Review becomes a Safeguarding Case Review. 

Each type of review in the type of review drop down section has converted following it. This is so we can identify how many Rapid Reviews actually become a Safeguarding Case Review. So when a rapid review becomes a safeguarding case review close the rapid review in the tracker open a new case and select the type of review, for example DHR (converted), and then use one of the free text fields to provide a reference or link between the 2 records. 


3. Media interest

If there is any media interest in this case type in yes or no.

4. Further updates

If you need to make any updates about this incident please select yes to record them

Use the update section to add any further updates about the incident you are recording. You will need to include the date you have updated the record and add any notes using the update section.

5. Reporting dates

Enter the date the report is signed off and published, together with a URL (web) link to the report if it is available. 

The date signed off is the date the chair of the independent review or panel signed off the report.

Use the free text box to add any further comments or updates, this may include information about child reviews, parental ACE, parental DA or parental mental health. You can not upload documents. 


6. Lessons learned and recommendations

Enter any lessons learned and recommendations under the organisation learning 1,2 and 3.  Each of the learning 1, 2 and 3 contain the same options.

Select yes or no if there is any media interest in the case. 


7. Reviewing your record

There is the opportunity to check your answers and amend these if necessary. If you need to change anything, please select ‘change’ on the right, and make the necessary amendments.

When you are satisfied with the answers, please select submit.

8. Completing the report

If the report is completed and signed off then the recommendations can be shared as all information is anonymised.

 

The tracker will be used to understand live cases because the local authority website will only have published closed cases.

The tracker is designed to replace your own local trackers as it will be live and information will be added when there are any updates on the case.  

All information entered into the Tracker must be anonymised, the tracker is not public facing, and there are restrictions on who can view records.
