Checklist data capture - questions for CHIS

These questions below can be used to capture data about your services.

  • How are you alerted to a change of address?
  • How do you receive the message, from whom and in what format?
  • Do you receive a movers in/out report? If yes, how often and who sends it?
  • Who processes the message and how do you process it (number of staff involved)?
  • What do you do with the data once received?
  • How long does it take to process the data?
  • How many messages do you receive per day?
  • Where does this information go to next? How do you notify the CHIS team?
  • What is the process for children born outside of area?
  • What happens to movers in and out?

Sample AS-IS map

This shows a sample AS IS process map for change of address. It is important to note that the change of GP would usually be notified in the same way as other demographic changes. Therefore, the change of GP process follows this process.

Sample TO-BE map

The embedded PowerPoint document below shows a sample TO BE process map for change of address. It is important to note that the change of GP would usually be notified in the same way as other demographic changes. Therefore, the change of GP process follows this process.

What benefits can be achieved?

This table table gives the benefits of change of address notifications in NEMS.

Do you currently? With NEMS Which means What may be impacted? Possible measures before and after
Receive address changes only periodically for example weekly or ad-hoc from multiple agencies You will receive address changes as they happen for all your population

You may know about a newborn moving in earlier

The accuracy of your population cohort is improved

Timeliness of processes for newborns (bloodspot)

Better support for commissioning and discussions and reporting

NBV, Hepatitis B, bloodspot KPIS

(see also birth notification)

Frequency of receipt of demographic changes per day or week

Ever find out a child has moved out some time after the event? As above Receiving CHIS/HV service may be notified sooner or more consistently proactively for a mover out Responsibility for child potentially re-allocated sooner with reduced risk of missing an intervention  Number of movers out initiated
