Newborn Infant Physical Examination (72 hour NIPE)

The Newborn and Infant Physical Examination (NIPE) screening programme screens newborn babies within 72 hours of birth, and then once again between 6 and 8 weeks for conditions relating to their:

  • heart
  • hips
  • eyes
  • testes

Only the 72-hour NIPE is available through NEMs interoperability and this relies on maternity units in your area using the NIPE SMART system (the majority of maternity units do). NIPE is usually carried out in hospital via maternity departments before the baby is discharged. Sometimes it is done at a hospital or community clinic, GP surgery, children's centre or at home.  The aim is to spot any problems early so any necessary treatment can be started as soon as possible.

Results will be made immediately available by the health professional carrying out the examination. If the baby needs to be referred for more tests, they will discuss this with the parents.

The results will be recorded in the baby's personal child health record (redbook).
In some parts of England, Health Visiting Services, which are no longer part of an integrated Community System with a CHIS module, may no longer receive the screening information/results directly. This will create a gap in the Health Visiting clinical record, leaving the health visitor blind to whether a child has screened positive for one of the nine conditions listed above (Blood Spot Screening). This message closes those gaps.

Messages received through this release will mean that health visitor will receive NIPE results automatically when published and ingest the results into the child’s record. Health visitor systems can then flag positive results to alert health visitor to children/families needing additional support. This will improve the availability of information to clinicians and improve the decision-making/effectiveness of interventions.

Outcome status testes Whether or not a problem was detected or suspected with testes. 
Outcome status heart  Whether or not a problem was detected or suspected with heart. 
Comment  Supporting text may be given covering regarding the screening test, outcome and actions taken. 
Checklist questions

  • How do you currently receive results for NIPE (72 Hours) screening into CHIS and by how many sources?
  • In which format do you receive the results (email, paper, spreadsheets, log on to screening system, smart4hearing).
  • How do you process the NIPE (72 hours) screening results? Do you rekey any results?
  • Do you report results to Health Visitors and GPs?   If yes, what, how, and how often.
  • How long does it take you to do this?
  • Are there any challenges with the current processes?
  • Do you ensure all children you are responsible for have a NIPE (72 hours) screening result? 
  • Do you get sent results for children not in your area and what do you do with them?  How do you ensure that children who do not have a NIPE (72 hours) test, are followed up?
