HMR101
This chapter describes how to create a HMR101 sample request form within NHS CSMS
It is not mandatory for a sample taker to create a HMR101 sample request form. Alternative systems are available to capture the sample details, and locally created forms can be sent with the sample to the laboratory.
The local laboratory recommended process should be followed.
Creating a HMR101 sample request form
To begin the process of creating a HMR101 sample request form, from the patient summary screen, select Go to HMR101.
Sample taker code
NHS CSMS does not validate sample taker codes; however, this information is required on the HMR101 form when sending it with a sample to a laboratory. The code entered into CSMS will populate on the HMR101 form.
Options for creating a new HMR101 sample request form
There Are 2 options for creating a HMR101 sample request form, it is recommended that a digital form be created.
Create digital form (recommended)
This option will pre-populate the form with the participants demographic details, and all entered sample and clinical details. This reduces the risk of incomplete or illegible information being received by the lab and the sample potentially being rejected on that basis.
Preview and print paper form
This option will prepopulate with the participants personal and demographic information only. The sample and clinical details will then need to be completed on the printed form by hand.
At any point whilst entering details to be prepopulated on the HMR101, there is an option to go back to the previous screen to re-enter or check the details entered.
Both options for creating the HMR101 form in CSMS still require the form to be printed and sent with the sample to the laboratory.
Episode address
The participants’ current registered address is displayed on the screen. The sample taker should confirm with the participant that these are the address details they want the test results should be sent to. If the participant wishes for their test results to be sent to an alternative address, this can be entered manually by selecting Send result letter to a different address. This will display a screen with fields to manually enter the alternative address
The address entered will only be used as the address to send the result letter for this test. It does not update the registered address on Personal Demographics Service (PDS).
