The final page asks you to confirm the information you have entered in your form and you should review the entries on the previous pages.
Before you submit the form, you will have the option to provide an email address to receive confirmation of your submission. Select Yes and provide a valid email address to take up this option.
If you select No you will not receive a summary of the responses completed in the submission.
Select submit to complete the form and submit the information for this contract. Once submitted you should see the following thank you message.
You should receive your confirmation email from ‘Snap Webhost’ with a summary of your return within 2 hours. If you don’t receive your confirmation it may have gone to your email junk or spam folder.
You must complete a KO41b return for each NHS Dental Contract you have held between the dates of 1 April 2024 and 31 March 2025. To complete the online form for another contract please use the KO41b 2024- 25 link and enter the relevant contract number into the online form.

Shortly after the deadline NHS England will be sent the information which will be published in late 2025.
