Current chapter – Submitting data using the NHSBSA online form


The following screen shots below were taken from a return that had 4 complaints. 


1. Access the online form and enter the contract number

To access the survey and submit your data follow the KO41b complaints survey link.

You will be asked to enter your contract number to start your submission. Your contract number is the contract ID as shown in Compass and will be 10 digits long with no spaces or special characters. 

Click on the next button, which will take you to a form summarising your contract number and the principal practice address. The first page of the form will open as follows:

You should check the contract information shown on this screen matches the contract for which you are entering the complaints. Do not proceed if the information does not match the contract you want to submit complaints for. If you need to complete the form for a different contract, please select click here to reset the form and start again.

If you do not recognise the address, please check Compass using the contract search function to see if the address shown matches another treatment location for your contract. If you are still unsure, email the NHSBSA for the address details that match the contract number.

If the contract details are correct select the next button to proceed to the guidance screen below.

How to complete the form

Data should be collected for all practices under each contract and completed in one return for that contract. To continue and complete the form select the next button.
